BREAST AUGMENTATION


Breast form and size are a very important factor for many women, as far as their appearance is concerned, self-confidence.

Breast hypoplasia is a condition of considerable familial causes that can even affect women's sexual and social life. To cure this problem breast augmentation surgery is the most successful surgery performed by plastic surgeons all over the world. In this operation, the expectation of each patient is different and the success of the operation can be quite dependent on the anatomy of the patient.

Breast augmentation surgeries are mostly performed with silicone implants, but also with fat injections in selected patients in recent times. Silicone implants are roughly divided into anatomic (drop-shaped) and round.

In the scientific studies conducted in this respect, neither of the anatomic nor round implants has superiority in terms of patient satisfaction. Therefore, the choice depends entirely on the patients expectation and the experience of the plastic surgeon.

Nowadays, previously used serum-filled implants are not been used as a result of long investigations that has been done by FDA (American Medical Association) and after the announcement of these studies that there are no side effects of silicone implants. The biggest post-operative risk of breast augmentation surgery is the capsule reaction. Apart from this, wound healing deficiency, infection, hemorrhage and hematoma. There is not even a slight effect on breastfeeding. Ultrasound should be preferred to follow-up. Pain and swelling are temporary surgery side effects.

Patients should be informed about the operation at least 3-4 days before surgery and a consent form must be signed. The consume of alcohol, garlic and blood thinner medications should also be discontinued before the operation (please speak to your Plastic Surgeon for the duration). The surgery should be done under general anesthesia.

I do the surgeries with the “Dr Satir Elevator-Retractor” which I developed and patented, and the surgery lasts 30-50 minutes. However, the experience of each doctor is different in this regard. The patient can one day hospitalised or at the same day discharged (according to the patient's condition).

There are 3 main incisions used during surgery:
1- The most commonly used submandibular incision (which I prefer)
2- Peri-Areolar incision
3- Axillary incision

We can put the implant under the breast gland or under the muscle. I prefer the most dual plan (between pectoralis major and the other muscles). However, we can also leave the implant under the complete muscles. Since I use Dr.Satir Elevator-Retractor I have a broader angle of view and can speed up the operation as well as I can control the bleeding in a better way. For this reason I do not use drainage (but this is my personal preference based on experience).

After the operation I usually use 1 compression bra and 2 Stuttgart Corsets. The patients must wear this bra and corsets on the first 6 weeks 24 hours a day and on the next 6 weeks 12 hours a day.

Patients are controlled on the 10 to 15th days(to remove stiches), 6th week, 12th week and 6th month after the surgery. After 3rd week of the surgery patients should start with a breast massage.

 


Gynecomastia Treatment (Breast Reduction in Men)


The breast form may become a nightmare for many men. It is a very important factor that can affect men's self-confidence, sports life, psychological health and success in business life. Gynecomasti is a condition related to and hormonal reasons. It can be seen up to 10% of all males. Before undergo breast augmentation (Gynecomastia) surgery, patients need to talk to their plastic surgeons on their own expectations and the possibilities of this operation. Gynecomastia surgery is not an easy operation. Complications such as infection, wound healing disturbance, numbness (also at the nipple) and scars may develop. After this surgery, symmetry may be better than before the operation however 100% symmetry will not be achieved.

Patients should be informed about the operation at least 3-4 days before the operation and a patient consent form must be signed. They should stop smoking or reduce the amount of cigarette consumption before the operation.

The consume of alcohol, garlic and blood thinner medications should also be discontinued before the operation (please speak to your Plastic Surgeon for the duration). The surgery should be done under general anesthesia. I personally use the Liposuction method, which is combined with partial breast tissue excision, following the half-moon-shaped skin incision around the nipple.

The operation lasts about 1-1.5 hours. However, the experience of each doctor is different. After the Gynecomasti operation, the patient can one day hospitalised or at the same day discharged (according to the patient's condition). There are several techniques that can be used during surgery. This is related to the degree of the patient's Gynecomastia. Except for the method I use, only liposuction or skin manipulation can be preferred in Gynecomasti surgery . I do not use drainage in large quantities (but this is my personal preference based on experience). According to the conditions of the patients, generally I do not use drainage and discharge on the same day. After the surgery, I usually use a compression garment. The patients must wear this compression garment on the first 6 weeks 24 hours a day and on the next 6 weeks 12 hours a day.

Patients are controlled on the 10 to 15th days(to remove stiches), 6th week, 12th week and 6th month after the surgery. According to the situation I can recommend a vacuumed massage.

In the long term after the surgery, the biggest problem may be numbness around the nipple.


Mastopexy (+Augmentation)


Breast form and size are a very important factor for many women, as far as their appearance is concerned, self-confidence. Breast ptosis can be seen as the result of weight loss and gain, post-pregnancy and hormonal changes (such as menopause).

Before breast ptosis surgery, patients need to talk to their plastic surgeons about their expectations and possibilities. Sometimes Augmentation surgeries can accompany these operations. Mastopexy surgery is not an easy operation. Complications such as infection, wound healing defficiencies, numbness (also at the nipple) and scars may develop. After this operation, symmetry may be better than before the surgery, but it will never be possible to achieve 100% symmetry. When we make Mastopexy and Augmentation surgery together the operation can be more difficult and more complications can occur.

Because of the high revision complication rates, some plastic surgeons are performing Mastopexy Augmentation operation in two sessions. Patients should be informed about the operation at least 3-4 days before the surgery and a consent form must be signed. The consume of cigarette, alcohol, garlic and blood thinner medications should also be discontinued before the operation (please speak to your Plastic Surgeon for the duration).

The surgery should be done under general anesthesia. The operation time lasts about 2 to 4 hours depending on the extra Augmentation surgery needed. However, the experience of each doctor is different.

After the Mastopexy operation, the patient can one day hospitalised or at the same day discharged (according to the patient's condition). I use the transverse incision technique during surgery (depending on the situation, inverted T incision can be). The choice of these methods is entirely based on the experience of the plastic surgeon.

If I do an additional Augmentation surgery, I use drainage, otherwise generally I don’t use drainage. After the operation I usually use 1 compression bra (if I do additional Augmentation Surgery I am using Stuttgart Corsets). The patients must wear this bra and eventually corset on the first 6 weeks 24 hours a day and on the next 6 weeks 12 hours a day.

Patients are controlled on the 10 to 15th days(to remove stiches), 6th week, 12th week and 6th month after the surgery. After 6th week of the surgery patients should eventually start with a breast massage.

After breast Augmentation surgeries silicon implants can eventually stay life time as long as there is no complication.

Complications such as infection, wound healing defficiencies, numbness (also at the nipple) and scars may develop. After this operation, symmetry may be better than before the surgery, but it will never be possible to achieve 100% symmetry.

There are several techniques that are used in Breast Reduction Operations. I preferably use “Superomedial Pedicule, Superior Pedicule, Lateral Pedicule and Inferior Pedicule” techniques during the operation. Each surgeon can choose one of these technique according to his/her experiences. I usually use the special technique of Dr. Findlay’s “Superomedial Pedicule” where I had the chance to work with him during the period I worked in Canada and seen the positive results of his abstracts that are publishes worldwide.

Due to my personal experiences generally I don’t use drainage. The patient can one day hospitalised or at the same day discharged as I do not use drainage (according to the patient's condition).

After the operation I usually use 1 compression bra (if I do additional Augmentation Surgery I am using Stuttgart Corsets). The patients must wear this bra and eventually corset on the first 6 weeks 24 hours a day and on the next 6 weeks 12 hours a day.

Patients are controlled on the 10 to 15th days(to remove stiches), 6th week, 12th week and 6th month after the surgery. After the surgery the common problem observed is the wound healing. Due to that reason I call my patients and see them frequently to make small adjustments if necessary. According to my experiences I did not receive any negative feedbacks in breast feeding or numbness so far.


ABDOMINOPLASTY OPERATION


Especially in this century, it has become very important to have a thin and slim waist with the influence of the media. Post-pregnancy excess skin and cracks are especially serious problems for women. Accompanying rectus diastasis (relaxation of abdominal muscles) sometimes makes this problem more prominent.

These changes might not be corrected with sport or liposuction, sometimes. In this case, abdominoplasty can be performed which is defined as muscular narrowing, in addition to skin fat tissue excision. In this surgery, not always, but often the position of the belly button is changed.
Although successful results can be obtained with abdominoplasty operations compared to preoperative condition, patients’ expectations might not be met when if expectations are too high. Patients should be conscious of scarring, in particular.

There are also risks of abdominoplasty surgery as in every operation. First of all, surgery should be done by an experienced plastic surgeon. Swelling, hematoma, long-lasting fluid accumulation (seroma), pain, infection and abirritation complaints are possible after the surgery. Significant scarring is one of the most important complications of surgery.

Patients needs to be well informed by the plastic surgeon before the operation. It is necessary for patients to stop blood thinners, discontinue (reduce) contraceptive and hormone medications for a while and give up smoking and alcohol for some time.

Surgery is performed under general anaesthesia in steril conditions.
Patients might stay in hospital for 1-2 days or also be discharged directly to their (on condition to drop by for daily control with drainage). Drainage may remain for 1-4 days after surgery. Patients should absolutely use corset for 6-12 weeks and walk in upright position for the first 2 weeks.

It is necessary to discontinue exercising for postoperative 6-12 weeks.
Minor changes can be done after 3-12 months from the surgery and repeating abdominoplasty operation is also possible.


BRACHIOPLASTY (ARM LIFT) OPERATION


Sleeve gastrectomy operations have become very popular with the prevalence of obesity. As a result of these surgeries, arm lifting operations have also become quite common.
Skin and fat excision operations can be performed for these changes about skin and sometimes with excess fat.

Although successful results can be obtained with arm lifting operations compared to preoperative condition, patients’ expectations might not be met when if expectations are too high. Patients should absolutely be warned before the operation especially about scarring.

LIPOSUCTION SURGERY


There are fat deposits in our body which don't disappear by sport or diet, especially depending on the genetic code structure. The procedure of removing these fat deposits with a special aspirator is called as liposuction.

The success of liposuction surgeries is closely related to the patient's age or skin elasticity. While non-existing of scarring is the advantage of this operation, some roughness and skin saggings that might occur after the surgery is the disadvantage.

Liposuction surgery can be performed under local anesthesia as well as under general anesthesia. The part of body from where fat removal will be performed is inflated by injecting a special solution before the surgery and then the liquid and fat in this region are absorbed by an aspirator. Besides conventional liposuction, there are also such methods as vibration assisted liposuction, laser ultrasound and radiofrequency liposuction. Although these methods are not very different from each other, laser and radiofrequency methods have been indicated to have a fair amount of skin lifting effect. In this case, these methods are more effective in patients with loose skin elasticity.

Liposuction surgery is a surgery of the plastic surgery department and I recommend it to be performed by a plastic surgeon. Side effects due to allergic reactions can be seen during surgery.
Seroma-hematoma (fluid or blood accumulation), sense of paraesthesia, edema (swelling), infections even rare, thrombosis, embolism and serious life-threatening situations due to visceral injuries by an inexperienced surgeon may occur after the surgery.

Apart from all these, a rugged skin appearance might be seen after the operation.
Patient should be pre-operatively enlightened about the procedure by the plastic surgeon and alcohol, smoking, blood thinners should be stopped before a certain time from the procedure.

Patient can directly return home after the operation. However, patient should wear a special compressed dress for 24 hours during 6 weeks and then 12 hours during 6 weeks. After surgery, it is necessary to give a break for strength sports for a while.
